Young female with fatigue, weight gain, cold intolerance

They are signaling to the hypo thyroidism condition,due to Hasimotos thyroiditis.

Cause: autoimmune disorder hadimotos thyroiditis,Iodine deficiency,post thyroidectomy or radiation therapy,drug induced, pist partam thyroiditis

Pathogenesis: auto antibodies attack thyroid tissue ,progressive destruction of thyroid,decreased T3,T4, production,increased TSH production

Risk factors:

Women >40 years

Postpartum women

Patients with autoimmune diseases

Individuals on lithium or amiodarone

Family history of thyroid disorder

Management:

Drug of choice: Levothyroxine (T4)

Dose: ~1.6 µg/kg/day (start low in elderly/cardiac patients)

Administration: Once daily, empty stomach, morning

Monitoring: TSH after 6–8 weeks, then 6–12 monthly

Target: Normal TSH (pregnancy—lower range)

Subclinical: Treat if TSH >10, symptomatic, or pregnant

Emergency: Myxedema coma → IV levothyroxine + hydrocortisone

Counseling: Long-term/lifelong therapy, strict compliance
