A ‘hot tooth’ is tricky because inflammation makes numbing difficult. I think they often need more anesthetic, specific injection or even sedation to truly block the pain.

A “hot tooth” refers to a tooth with pulp inflammation that causes severe pain, even after a standard anesthetic block. This condition commonly occurs in lower molars and often requires a combination of blocks to achieve numbness.

A hot tooth is a tooth with inflammed pulp which is difficult to anesthetize.
The causes are the presence of tetrodotoxin resistance channels in the pulp, also a thick cortical mandibular bone, it basically acts as a barrier for anesthesia, or even when ionized form of LA isnt available and low tissue pH.
Management- pre medication with NSAIDs, corticosteroids, nitrous oxide, ketamine, benzodiazepines, other supplementary techniques help too.

A “hot tooth” is a tooth that’s difficult to anesthetize due to inflammation or infection.
Increased blood flow and acidic environment reduce anesthetic efficacy.
Nerve sensitization makes the tooth more responsive to stimuli.
Supplemental anesthesia techniques may be necessary.
Additional anesthetic doses or different solutions may be used.
Pain management strategies, like pre-op pain meds, can help.
Effective pain management ensures patient comfort during treatment.

A hot tooth is actually difficult to diagnose as well. It take experience to really know more. You don’t know if it’s a hot tooth or the patient tells the pressure felt as tooth. If it really is hot tooth multiple approaches of LA can be done.
