Great question. Think of systems first: Causes can be respiratory, cardiac, metabolic, CNS/ neurological and then miscellaneous.
This will lead us to differentials. Obviously, think of common causes first: respiratory causes are the most common in this clinical vignette.

β’ Respiratory Distress Syndrome (RDS) β surfactant deficiency
β’ Pneumonia/Sepsis β perinatal infection
β’ Transient Tachypnea of the Newborn (TTN) β delayed lung fluid clearance
β’ Pneumothorax β air leak
β’ Congenital anomalies β e.g., diaphragmatic hernia
Initial management:
β’ Stabilize airway and breathing β warm environment, oxygen, CPAP/ventilation
β’ Supportive care β fluids, thermoregulation
β’ Investigations β CXR, blood tests, ABG
β’ Start empirical antibiotics if infection is suspected
β’ Give surfactant if RDS is likely
