Neonatology Quiz: Let me know the Answer

RDS is a disease of prematurity. Tell me a few conditions when RDS can occur in full term (FT) babies.
And do not Google it.

RDS can happen in full-term babies in some cases like infants of diabetic mothers (due to delayed surfactant), C-section without labor (no stress-induced steroid surge), birth asphyxia, genetic surfactant protein defects, or infections like sepsis that interfere with surfactant.

Delivery complications, delayed labor and delayed formation of lung surfactant can be a few reasons for the same.

Also low birth weight can be another reason.

Respiratory Distress Syndrome (RDS) typically affects premature infants due to surfactant deficiency.

However, full-term babies can develop RDS under certain conditions, such as:

  1. Maternal diabetes: Increased insulin levels can affect surfactant production.
  2. Cesarean delivery without labor: Reduced stress of labor may impact surfactant release.
  3. Perinatal asphyxia or meconium aspiration: These conditions can cause lung injury and impair surfactant function.
  4. Genetic surfactant deficiency: Rare genetic mutations affecting surfactant proteins.

These conditions can increase the risk of RDS in full-term infants.

All are valid causes. Do remember MAS (mec aspiration syndrome) as an important cause of RDS in term babies.
Also as we know MAS essentially is a disease of term babies (not in preemies).

  • infants of diabetic mother
  • C section without labour
  • perinatal asphyxia
  • meconium aspiration syndrome
  • genetic surfactant protein deficiencies
