Cough in 6 weeks old baby- what is the probable Dx?

A 6 weeks male baby born vaginally has history of runny nose and staccato cough since one week. No history of high grade fever.
What is the probable diagnosis and how would you diagnose it?

This can be “Chlamydia trachomatis pneumonia”

Diagnosis:
PCR/Culture from nasopharyngeal swab
Chest X-ray:Bilateral interstitial infiltrates
Eosinophilia on blood smear

Treatment:Oral Azithromycin or Erythromycin for 14 days

The probable diagnosis for a 6-week-old male infant with a runny nose and staccato cough for one week, without high-grade fever, is pertussis (whooping cough). This is suggested by the characteristic staccato cough, especially in a young infant. To diagnose it, I’d recommend a clinical evaluation combined with a nasopharyngeal swab for PCR testing to detect Bordetella pertussis DNA, which is the most reliable method. Additional tests like a complete blood count might show lymphocytosis, supporting the diagnosis. Prompt diagnosis is key due to the risk of complications in this age group.

And beware of Erythromycin causing hypertrophic pyloris stenosis in neonates.
