When Black Stains on Children’s Teeth Aren’t Just About Poor Brushing

As dentists, we often see parents worried about black stains on their child’s teeth, assuming it’s due to poor oral hygiene. However, not all discoloration is caused by inadequate brushing.

One interesting cause is chromogenic bacteria. These bacteria interact with iron present in saliva or gingival fluid, producing dark pigments that appear as black lines or dots, usually along the cervical margin of primary teeth. Although these stains can be aesthetically concerning, they are generally not associated with active dental decay and are often seen in children with relatively low caries risk.

Professional scaling or polishing can remove the stains, but recurrence is common. Instead of blaming oral hygiene alone, it is important to educate parents about the nature of these stains and encourage regular dental visits for monitoring and maintenance.

Understanding the difference between pathological conditions and harmless clinical findings helps us provide better patient education and avoid unnecessary anxiety.

I didn’t know chromogenic bacteria could cause these stains. It’s a great example of why evidence-based counselling can make such a difference for both parents and children.

Explaining that these stains are generally benign, recurrent, and not necessarily a sign of poor brushing can actually improve compliance and trust.

This is one of the most common issues that parents of pediatric patients come up with. Due to lack of proper oral hygiene maintenance, these chromogenic bacteria produce pigments that cause stains, especially on the cervical portion of the crowns.

To begin with I would explain them the reason behind the discoloration and ensure them that it can be removed by scaling and polishing. Additionally I would counsel both parent and the child on oral hygiene practices.

Interestingly, some parents mistake chromogenic stains for dental caries because of their appearance. Explaining that these are simply extrinsic stains and not cavities often brings them immense relief.
