PGY1 is called internship in the USA. Lot of hard work, scut work and longer duty hours (better now as more regulations have kicked in).
This, just be ready for a strenuous one year- this mindset helps sail through first year. Familiarize yourself with clinical work, protocols and general functioning of the units (ward), computer system (EMR) and such related.
As a first-year PG resident, some biggest challenges include:
