We all use toothbrush covers for travel or to “protect” our brushes from bathroom aerosols, but as healthcare professionals, it’s important to look at the microbiology behind the plastic.
The Common Belief
Most patients use toothbrush covers to prevent “toilet plume” (the aerosolization of microbes during flushing) or to keep bristles clean in a travel bag. While the intention is hygiene, the execution might be counterproductive.
The Clinical Perspective

The Moisture Trap: Microorganisms—especially bacteria like Staphylococci and coliforms—thrive in moist, dark environments. A snap-on cover creates a mini “greenhouse” effect, preventing the bristles from air-drying and promoting biofilm formation.

The “Toilet Plume” Reality: While faecal coliforms can be found on toothbrushes in shared bathrooms, studies suggest that rinsing the brush thoroughly and allowing it to air-dry is often more effective than sealing it in a damp container.

Air-Drying is King: Store brushes upright in an open area, ensuring they don’t touch other brushes.

Ventilation Matters: If using a cover (for travel), choose one with multiple ventilation holes.

Close the Lid: Advise patients to close the toilet lid before flushing to significantly reduce aerosol spread.

Strategic Storage: The simplest way to avoid contamination is to store the oral hygiene kit outside the washroom entirely.

Replace Regularly: Remind patients to swap their brush (and their cover!) every 3–4 months, or after a viral illness.
