Gas Gangrene: A Clinical Case That Changed My Perspective on Infection Control

Today, I would like to share an experience with the Med Bound Hub community from my 2nd year hospital posting that will stay with me forever in my life.

As a 2nd year Pharm D student, I was posted in the general women’s ward, and there I came across a case of gas gangrene. Now, gas gangrene is a rare, rapidly progressing bacterial infection that destroys muscle and soft tissue, often producing gas and toxins, and can be life-threatening.

This disease had affected the patient’s entire lower back, some parts of the upper back, and was spreading across her legs as well. In the ward, she had layers of dressings done as the bacteria were destroying her muscles rapidly, and the medications were doing their action at a rate slower than the spread of the infection.

The thing that struck me the most was that this infection was caused in her lower back and hip region due to the administration of a non-sterile injection of PCM, which she received from a physician when she had visited for a fever a few days back. This girl was in her mid-teens, young and vibrant, but succumbed to the mistake of another.
It made me realise that improving healthcare is not only about advanced ICUs or expensive medicines.
Sometimes it begins with basic sterility, accountability, and awareness at the primary care level.

This made me question whether our country is still facing a shortage of a good healthcare system and proper treatment. And where exactly are we lacking to make the changes and build a solid foundation towards healthcare?

We might have expensive medicines and ICU facilities but the basic sterilization protocol is lacking in some healthcare centres. We need to follow strict sterilization protocols, be it a small clinic or a big hospital.

We have to update our knowledge daily and have to take precautions while giving services to our patients.Patient care is the most important duty of all doctors.

Gas gangrene due to IM injection is rare but possible. While it can occur in healthy individuals, causes of compromised immune system must also be ruled out. At this day and age, it is sad to see such a thing happen due to lack of sterilization.

Sterilization must be followed at all levels of healthcare. A small lapse like this can have devastating consequences.

Gas gangrene progressing from something as routine as an injection highlights how critical infection control is at the primary care level. Prevention is often more powerful than treatment.
