Bluish discoloration around umbilicus?......is this 🍌 Banana belly?

When there is intra - abdominal bleeding then there is bluish discoloration around umbilicus.
This is called # Cullen sign

Acute pancreatitis
Ruptured ectopic
Trauma

Can be seen in conditions like :-

  1. Acute Hemorrhagic Pancreatitis
  2. Abdominal trauma
  3. Ruptured Abdominal Aortic Aneurysm (AAA)
  4. Ruptured ectopic pregnancy
  5. Perforated peptic ulcer
  6. Liver diseases with bleeding
  7. Rectus Sheath Hematoma
  8. Coagulopathies

Can it be confused with blunt trauma? What would confirm it as cullens sign?

Blunt trauma is definitely a DDx. But,history and local exam and further investigationa should help us reach the diagnosis.

The bluish discoloration around the umbilicus is likely Cullen’s sign, which can indicate intra-abdominal bleeding or pancreatitis. It’s not related to “Banana belly,” which doesn’t seem to be a medical term. Cullen’s sign requires prompt medical evaluation.

Cullen sign indicates intra-abdominal bleeding causing periumbilical bruising.
Common causes: acute pancreatitis, ruptured ectopic pregnancy, abdominal trauma, ruptured abdominal aortic aneurysm, and hemorrhagic intra-abdominal conditions.

It releases powerful enzymes that can digest nearby blood vessel walls, causing them to leak or rupture and leads to bleeding or fragile pseudoaneurysms that burst.
