Zilebesiran: A First-in-Class RNAi Therapy for Hypertension

Zilebesiran is an investigational small interfering RNA (siRNA) therapy developed by Alnylam pharmaceuticals for mild to moderate hypertension.

:right_arrow: M.O.A: It targets angiotensinogen (AGT) in the liver, suppressing the renin–angiotensin–aldosterone system and lowering blood pressure.

:right_arrow: R.O.A: Injected subcutaneously, it offers long-acting control with dosing every 3–6 months.

:right_arrow: Results of phase 1 trials: Single doses ≥200 mg produced sustained systolic blood pressure (SBP) reductions (10–20 mmHg) for up to 24 weeks with good tolerability.

:right_arrow: Results of phase 2 trials (KARDIA): Doses ranging from 150–600 mg reduced SBP by ~10–20 mmHg versus placebo, both as monotherapy and add-on therapy. Main side effects were mild injection-site reactions.
