What Practicing Dentistry Taught Me in Both Metro Cities and Small Towns

After practicing dentistry in different settings — whether in tier-1 cities, tier-2 cities, or small towns — one important thing I have consistently observed is that everything ultimately comes down to awareness.

In many small towns, dentistry is still largely pain-driven. Until patients experience severe pain or discomfort, they often do not feel there is any problem in their oral cavity.

I have seen patients with severe Oral Submucous Fibrosis who continued ignoring the condition until their mouth opening reduced drastically or they experienced intense burning sensations while eating. For them, difficulty was normalized until it started interfering with daily life.

On the other hand, in tier-1 and many tier-2 cities, dentistry is rapidly shifting toward preventive and cosmetic care. Patients are aware of small cavities, regular scaling appointments every six months, and the importance of maintaining oral hygiene before pain even begins.

But the difference goes beyond awareness of disease — it also reflects changing perceptions of appearance and confidence.

Patients in metro cities are conscious about how their smile looks, how aligned their teeth are, and how dental aesthetics affect their overall personality. Many patients directly ask about Invisalign or smile designing because they already know the available treatment options.

Meanwhile, in many smaller towns, people are still hesitant or even fearful about orthodontic treatment. Crooked teeth are often not considered a problem unless they cause pain or functional difficulty.

This contrast made me realize that dentistry in India is not just divided by geography — it is divided by education, exposure, accessibility, and priorities.

As dentists, we are not only treating teeth. Sometimes, we are also bridging the gap between awareness and neglect.
And perhaps that is one of the biggest responsibilities of healthcare in a country as diverse as India.

Are we only treating teeth differently across India, or are we treating awareness differently too?

I think in Urban areas awareness about dental treatment is more. Specially in metro cities , Dental aesthetics is popular. People are very conscious about like you said crooked tooth. On the other hand, in rural areas people don’t see it useful .

This mentality should be changed. Because as other part of body, oral health is also very important.

In urban areas, people are educated and conscious about their status but in rural areas, the awareness become more important because they think about dentist when they face any tooth related problems.

This is a sad truth of the current state of our country. As a dentist who has experienced the same situation, I can relate to this a lot. I feel more intivative should be taken to improve dental awareness all over the country.
