Tacrolimus- A Safer Alternative to Topical Corticostroids

Corticosteroids have been most commonly used for the treatment of atopic dermatitis/eczema for ages. Although steroids work effectively in improving the symptoms of atopic dermatitis, like itching, rashes, and inflammation of the skin, its long term use can lead to skin atrophy or thinning of the skin.

However, a good alternative to corticosteroids, called Tacrolimus, has been studied that not only works effectively on atopic dermatitis symptoms but is also safer than steroids as it does not cause skin atrophy.

A study published in 2023 in the journal Frontiers in Pharmacology found that Tacrolimus is much safer than corticosteroids. Let me brief about that study. It was done on 200 children with eczema. One group was treated with Hydrocortisone, which is a mild steroid, and the other group was treated with Tacrolimus for 3 weeks. The results showed that the group treated with Tacrolimus showed good improvement in symptoms and did not cause any skin atrophy compared to Hydrocortisone. This is a remarkable study. I believe extension studies should be performed. Once long-term effects are proven, it can be implemented.

If tacrolimus doesn’t have it’s effect on bones , then it would be boon for patients, as long-term usage of steroids would affect bone density..

Corticosteroids are the first drugs that come to mind when it comes to autoimmune diseases. How ever Immunosupresive drugs like Tarcolimus should be kept in mind while prescribing medications.

Tacrolimus mainly works by decreasing the production inflammatory markers in our body. If the side effects are low these drugs can be used in the management of various inflammatory disease.

I think tacrolimus topical ointments are used for a variety of skin ailments apart from atopic dermatitis / eczema like mentioned in this post . Tacrolimus is also useful to treat skin discolorations caused due to vitiligo or other chemical burns on skin . I remember reading an article where it’s advised to apply tacrolimus in a thin layer on affected white spot area and expose it to sunlight for approximately 15 minutes for it to activate and help in melanin production in that specific area.

Tarcolimus inhibits calcineurin, suppressing T cell activation and inflammation. its considered safe for using long term because it doesn’t cause skin atrophy.whereas other topical corticosteroid like hydrocortisone used long term leads to skin atrophy.

@Snazir Yes, this study was done for 3 weeks only. However, there are some studies that have proved that it is much safer than corticosteroids in the long run.

Good research! Tacrolimus. We should remember its name when prescribing corticosteroids to patients. If it is treated well, then there is no need to use steroids, as they can have many side effects when used long-term.
