Postpartum Renal Stones: Calcium Supplements and Hidden Risks

Pregnancy and postpartum are periods of immense physiological changes, and sometimes these changes quietly increase the risk of kidney (renal) stones. While calcium supplementation is essential for bone health of both mother and baby, excessive intake, combined with dehydration or a personal history of kidney stones, can contribute to stone formation after delivery.
Why It Happens
During pregnancy, hormonal and metabolic adaptations increase calcium absorption.

After delivery, continued high-dose calcium supplements, limited fluid intake, and changes in urinary pH may lead to calcium crystallization in the kidneys. Symptoms such as flank pain, blood in urine, or recurrent urinary tract infections are common warning signs.

Survey Insights
A meta-analysis of 4.7 million pregnancies found that renal stones occurred in approximately 0.49% of pregnancies, highlighting that stones are rare but clinically significant. (Kirubarajan et al., 2022)
Population studies show that the risk of first-time kidney stones peaks 0–3 months postpartum, suggesting the period immediately after delivery is critical for monitoring.
U.S. national survey data indicate that women with a history of pregnancy have a higher prevalence of kidney stones (7.5%) compared with women who have never been pregnant (3.2%).
Prevention Tips
Maintain adequate hydration throughout postpartum
Follow recommended calcium doses; avoid excess supplementation
Monitor high-risk patients with a personal or family history of kidney stones
Seek early medical attention if symptoms appear
