Pharmacogenomics : Can Our Genes Decide Which Medicine works Best?

Have you ever wondered why the same medicine works well for one person but causes side effects or has little effect in another? As pharmacy students, we often learn about a drug’s mechanism of action, dosage, and adverse effects. But one question that caught my attention while studying pharmacology was: Can our genes influence how a medicine works?

The answer is yes, and that’s where pharmacogenomics comes into the picture.

Pharmacogenomics is the study of how an individual’s genetic makeup affects their response to medicines. Our genes can influence how quickly a drug is absorbed, metabolized, or eliminated from the body. This means that two patients receiving the same medicine at the same dose may have completely different outcomes. One may achieve the desired therapeutic effect, another may experience toxicity, while a third may not respond to the treatment at all.

A well-known example is clopidogrel, an antiplatelet medicine. Some individuals have variations in the CYP2C19 gene, which reduces their ability to convert clopidogrel into its active form. As a result, the medicine may not provide adequate protection against blood clots. Another example is warfarin, where variations in genes such as VKORC1 and CYP2C9 can influence dose requirements and increase the risk of bleeding if not considered.

Learning about pharmacogenomics changed the way I think about pharmacology. It made me realize that prescribing medicines is not just about selecting the right drug, it is also about selecting the right drug for the right patient. As research advances, pharmacogenomics has the potential to support more personalized treatment, reduce adverse drug reactions, and improve therapeutic outcomes.

Although genetic testing is not yet a routine part of healthcare for every patient, its role is steadily expanding. As future pharmacists and healthcare professionals, understanding the basics of pharmacogenomics will help us appreciate the shift from a “one-size-fits-all” approach to personalized medicine.

Pharmacology is evolving beyond understanding how drugs work. It is now helping us understand why drugs work differently in different people. I believe pharmacogenomics is one of the most exciting developments in modern medicine, and it has the potential to transform the future of patient care.
