While considering USMLE and a path to come to the US to pursue a medical career, keep in mind that getting ECFMG certification is just one step. Anyone who has cleared step 1 and 2 of USMLE, will be awarded ECFMG certificate.
After that one has to apply for medical residency interviews which are pretty competitive and the IMG / FMG has to come to the US to appear for the interviews (visa requirements).
And then the match happens conflicted by NRMP. You get into residency only if you match.
Then the game of coming here for joining the residency on a visa - J1 or H1B. Note that J1 visa is not a preferred visa.
