As a PharmD student, when I look at case sheets or prescriptions, I often focus on the drug name, dose, and frequency. But I think a prescription can tell us much more about a patient’s condition and the prescriber’s treatment approach.
When evaluating a prescription or drug notes in a case sheet, we can consider and identify:
Is the dose appropriate for the patient?
Are there any drug–drug interactions?
Does the patient have any allergies?
Does the patient understand the instructions clearly?
For example, when I observed one case sheet containing multiple medicines, I checked the drug content of the medicines and found that one drug was present in two different medicines in different combinations. This made me realize that the role of a healthcare professional is not just to identify medicines but also to evaluate the medication regimen carefully.
Conclusion: A prescription or case sheet should not simply be read; it should be analysed twice or thrice.

Yeah, a prescription can tell a lot about patient’s condition and how the patient is taking a particular drug, it should be accurate and correct as per the condition, the most important thing that i read is the doctor’s approval or statement and the dosing

I feel a prescription is the blueprint of a patient’s treatment plan. Every time I review one, my attention goes to the medications the patient is taking, especially the dosage. Because, dosage often provides valuable insight into the severity of the condition and potential considerations that may influence the further treatment plan.

Well said mam. I am also a pharm d student The first thing I always check is whether the prescribed medicines are appropriate for the patient’s diagnosis, followed by the dose, possible drug interactions, and any documented allergies. It helps ensure the treatment is both safe and effective.

Very rightly siad. A prescription is much more than it appears. Every drugs in the prescription should be evaluated properly. because every drugs has different pharmacokinetics and pharmacodynamics.

yes you are correct, some people use medications from multiple doctors at same time, where there is a chance of having the same medications in different combinations which has to be checked and taken care of.

Yes definitely! Prescription is not only about the medicines it’s about patient condition and their treatment plan. I thought, along with reviewing the prescription patient counseling is also needed. Even the best treatment fails of patient doesn’t understand how to take correctly.
