Most of us have heard this line:
“Antibiotics don’t work on viruses.”
That statement is absolutely true.
So then why does it sometimes feel like antibiotics help during viral illnesses — and why do doctors still prescribe them in certain situations?
The answer lies in what antibiotics are actually treating — and what they’re not.
Firstly,
Antibiotics do not kill viruses.
They are designed to target structures that viruses simply don’t have, like:
Bacterial cell walls
Bacterial ribosomes
Bacterial metabolic pathways
Viruses use our own cells to replicate, so antibiotics have nothing to attack.
Then why are antibiotics given during viral infections?
Because viral infections often open the door to bacterial infections.
When a virus damages the respiratory tract or weakens immunity, bacteria can take advantage. This is called a secondary bacterial infection.
In these cases, antibiotics are not treating the virus — they are treating the bacteria that followed.
A common example would be Influenza.
Influenza (flu) is caused by a virus.
Antibiotics do nothing to the flu virus itself.
But flu can damage lung tissue and lead to secondary bacterial pneumonia, often caused by:
Streptococcus pneumoniae
Staphylococcus aureus
Here, antibiotics are prescribed to treat the bacterial pneumonia — not the flu.
This is where confusion usually starts.
Another reason antibiotics may seem to “help”
Some antibiotics (especially macrolides like azithromycin) have anti-inflammatory effects.
They can:
Reduce airway inflammation
Modulate immune responses
So symptoms like cough or chest discomfort may improve — but again, this is not antiviral action.
This is also why antibiotics were explored (and largely ruled out) during early COVID-19 trials unless bacterial infection was suspected.
Viruses that definitely need antivirals, not antibiotics
For these infections, antibiotics have no role at all unless there’s a bacterial co-infection:
HIV → treated with antiretroviral therapy
Hepatitis B & C → treated with antiviral agents
Herpes viruses → treated with acyclovir-like drugs
Influenza → antivirals like oseltamivir (in specific cases)
COVID-19 → antivirals in selected patients
Using antibiotics here without indication only increases resistance and side effects.
Antibiotics don’t work on viruses.
They work around viral infections — by treating bacterial complications or reducing inflammation in specific situations.
Understanding this difference is important, because unnecessary antibiotic use:
Doesn’t speed viral recovery
Increases antibiotic resistance
Exposes patients to avoidable side effects
The next time antibiotics are prescribed during a “viral illness,” the real question isn’t
“Why antibiotics?”
but
“Is there a bacterial problem hiding underneath?”
