CQI Forms – Small Notes, Big Impact

During my internship, I learned that hospital safety isn’t automatic—it’s built step by step. One tool that surprised me with its importance was the Continuous Quality Improvement (CQI) form.

Whenever an error or near-miss is spotted, it gets recorded. Not to blame, but to fix the system. For example, I once saw an antibiotic order missing the dose and frequency. Instead of just correcting it, I also filled a CQI form so the gap wouldn’t repeat.

The process is simple: notice → document → submit → review → improve. But the outcome is powerful. It prevents errors, strengthens workflows, and protects patients.

As interns, filling these forms may feel small, but they create lasting change. One note today can prevent harm tomorrow.
