- Preventing Medication Errors
Clinical pharmacists review prescriptions for correct drug selection, dose, frequency and route of administration. By identifying prescribing errors, duplicate therapies and illegible orders, they help prevent errors before medicines reach the patient.
- Managing Drug Interactions and ADRs
Patients, especially those with multiple comorbidities, are at high risk of drug–drug interactions and adverse drug reactions (ADRs). Clinical pharmacists assess medication profiles, monitor for side effects and support early detection and reporting of ADRs, improving overall treatment safety.
- Medication Reconciliation
During hospital admission, transfer and discharge, discrepancies in medication lists are common. Clinical pharmacists perform medication reconciliation to ensure continuity of therapy and prevent omissions, duplications or unintended changes.
- Patient Counseling and Adherence
Effective counseling helps patients understand their medicines, dosage schedules and warning signs. This improves adherence, reduces misuse and empowers patients to participate actively in their own care.
- Supporting the Healthcare Team
Clinical pharmacists provide evidence-based drug information to doctors and nurses, assist in dose adjustments (especially in pediatrics, renal and geriatric patients) and contribute to antimicrobial stewardship programmes.
