This kind of error is due to insufficient knowledge and training in healthcare. It includes:

  • Mistakes in instructions and medication dosage
  • Insufficient sharing of treatment information with specialists or physicians
  • Poor documentation

  • Proper training and guidance for everyone. Effective guidelines should be created for every medical condition.

Yes true, miscommunications leads to many medical errors as information is not transferred properly. Detailed analysis of every medication, treatment to be given, history of patients by all the members should be checked and should be mentioned and stored in paper form.
