Regenerative Endodontics 🧬

Regenerative endodontics is a method of therapies that attempt to substitute damaged or necrotic pulp with tissue that is alive and able thereby, restoring normal physiological functions including immune defense, sensation and further root development in immature teeth.

Involves:
• Stem cells (from the apical papilla or from dental pulp itself)
• Scaffold materials (such as blood clots, platelet-rich fibrin or biomimetic hydrogels)
• Growth factors like BMPs and TGF-β
• Sealing off access with the use of a biocompatible material like MTA or bioceramics

Current Scenario

-The research suggests:
• These procedures are able to stimulate root maturation in the non-vital immature tooth
• There is thicker dentinal wall and longer root length.
• One cannot occasionally see indications of vascularization and reinnervation.
• It is very viable, especially for young patients.

Nonetheless, one needs to look into predictability and long-term outcome. There is no universal protocol and success rates differ.

Challenges and Controversies:
• Are we seeing a true pulp re-generation or is it just revascularization with fibrous tissue?
• Disinfection protocols can even negatively affect stem cell survival (triple antibiotic paste vs calcium hydroxide)
• Difficulty in obtaining apical bleeding along with its correct scaffold formation
• Interindividual variations
• Small number of clinical studies of a standardized methodology

Is It the Future?

Yes ; but cautiously. Regenerative endodontics heralds a new era in form and function as it no longer compromises active biological healing for passive repair. As research grows and universal and predictable protocols emerge, biologically based therapies might become common in place of conventional root canals, especially in pediatric and adolescent dentistry. As of now, it is too immature to overcome classical RCTs. However, it has ceased being a myth already. It is fast becoming a reality with research backing and hence may be on its way to becoming the mainstay of conservative endodontics.

It is one of the revolving field in dentistry as it makes immature teeth to mature teeth.

The potential of the pulp to revascularize is good, but it entirely depends on the criteria of proper case selection.
