It requires a thought full and central approach to deal with such conditions. Health care providers should take time to listen the patient for better mutual understanding of the disease and most importantly follow up to patient stay on track with their treatment plans.

For chronic disease management:
•Empathetic approach towards patient.
•Care for them.
• Make them understand about the disease, treatment and prognosis.
•Make them comfort in their situation.
•Make them understand that will be aside them for any help.
•Tell them to follow medication correctly.
•Maintain follow-up check up.
•Follow the diet and exercise whatever doctor recommended.

  • use shared decision making thus building trust and rapport
  • simplify the treatment plan
  • follow up actively
  • reinforce positives but celebrating small wins
