Intensive care unit is a complex place where patient receive various drugs. While medicines save life , medication error is still a major problem.
Medication errors may occur while prescribing, administering or dispensing a drug.
They are preventable , where errors are reported, analyzed and learned from, we can significantly reduce harm and improve outcomes in critically ill patient.
There are various medication errors that arises but the most common causes include
Polypharmacy
Communication gaps
Workload
High alert medications
Look alike-Sound alike drugs
Medication error occurs but they are preventable.
We can prevent such errors through various stratergies like
Proper monitoring of medication administration
Effective communication
Double verification system
Regular training and education for the staffs
Clinical pharmacist involvement in dose adjustments, interaction screening, reconcillation.
Thus in ICU, where every second and every dose matters, preventing medication errors is not an option, it is a responsibility.The main thing we should always remember is “Right drug, Right dose, Right time, Right patient, Every time”.
Precision, teamwork, critical thinking and vigilance saves lives and every lives matters.
