It is a broad question. But probably latent depression, and psychosomatic conditions.
Stress too is probably ignored by patients as well as medical professionals because we tend to normalize stress.

According to clinical studies PCOS in women is the most underdiagnosed condition.
The reasons are many such as:
- lack of awareness
- non specific symptoms such as irregular periods, weight gain, mood changes which are often dismissed as normal
- overlap with other conditions such as hypothyroidism
- misdiagnosis as hormonal imbalance.

Mental health conditions are often under diagnosed in my opinion due to :
- Stigma and social taboos surrounding mental illness can prevent individuals from seeking help.
- Lack of awareness and education about mental health can lead to misattribution of symptoms.

- Variability in symptoms and presentation can make diagnosis challenging.
