Good Documentation Practice (GDP) is a set of standards that ensures all records in healthcare are accurate, complete, consistent, and traceable. It’s essential for patient safety, regulatory compliance, and quality assurance.
Key Principles of GDP
- ALCOA+ – Data must be:
Attributable → Who performed the task?
Legible → Clear, readable entries.
Contemporaneous → Record at the time of activity.
Original → First record or certified true copy.
Accurate → Correct and reliable.
Plus: Complete, Consistent, Enduring, and Available.
- Clarity & Accuracy
No overwriting; if corrections are needed, strike a single line, sign, and date.
- Timeliness
Documentation must be done immediately after the activity, not later.
- Accountability
Every entry must have a signature/initials and date.
- Traceability
Records should allow tracking back to the person, time, and reason for the activity.
