Yes I’ve the experience because there are many companies that makes same medicines but some doctors only gives the best in the prescription that sometimes only available at nearby pharmacy centre so I think we should review before taking it from another medical because sometimes it’s duplicate or sometimes they have another content. I think after discharge the patient parents or anyone with them should discuss about this with the doctor.
Indeed, patients do require a great deal of assistance post discharge. Could this problem be combatted by more frequent and affordable post discharge summaries?

Well said , I also have the same opinion during treatment we all are focused to cure the disease but after taking care of all the required things Some do not check the discharge paper correctly which leads to more complications sometimes.One should be responsible for each and everything written on discharge papers to checked and if needed to be considered .
During my intenship, I came across a discharge summary where the IV drugs have not converted to oral for discharge and also the sodium levels, eventhough are normal for the patient, sodium correction was still written in the discharge meds.
The letter of discharge do have a impact on the patients’ security at home. All the details should be double-checked, that won’t lead to any confusion to the caretaker of the patient or the patient themselves at home.

I haven’t had the chance to review a discharge summary yet, but I’ve learned how important it is. Even one small mistake in meds can cause big issues later. It’s definitely something I want to be careful about in the future.

we have to be more careful about this its important for us we must have provide right instructions about doses and reviews the medicines
As a physician, I always make sure that the patient understood the regime. I would always write the instructions in easy language for them, preferably on the medicine strips and packets.
