It’s a question many patients bring to the clinic that “All reports are normal, so why does it still hurt?”. Does the absence of evidence mean the absence of disease, or are we limited by the current diagnostics ?
Conditions like fibromyalgia, functional disorders, or chronic pain syndromes often present with significant discomfort despite minimal or no detectable deviations on routine tests. Pain perception is also influenced by the central nervous system, emotions, past experiences, and psychological stressors alter pain processing.
This raises an important clinical and ethical question: how should we respond when tests are normal but the patient continues to suffer ? Dismissing the patient’s experience and even over-investigation can lead to frustration. Perhaps we need to learn to listen more carefully and do a through history taking.
Have you ever encountered a situation clinically or personally where pain was present but no clear cause was found ?
Yes..many time it’s not evident from reports but clinically patient suffering from it..and it needs attention of physician as well..but mostly physician ignores that and consider them as psychological.
I’ve seen that normal reports don’t always mean the patient is completely fine. Many times patients genuinely suffer even when tests are normal. In such cases, proper history taking, listening patiently, and understanding stress, lifestyle, sleep, and mental health also become very important. Treating the patient, not just the reports, matters a lot.
Yes. Pain may be the first signal body gives that there is something wrong going on inside the body. May be after a long time you come to know that the pain, that I had earlier was start of so and so disease.
Physician’s insight into the patient’s history & non verbal cues will help to differentiate & understand the psychological & physiological pain and able to manage accordingly.
This happens commonly. Patient comes to clinic with large bundle of reports and files. All reports are normal. Doctor also feels inspite of all normal reports why this patient is complaining. These patients are referred to psychiatrist or some additional lab is advised.
You have come up with good topic for discussion. Physician should be considerate with these patients and listen carefully to their complaints.
Yes, I was experienced same situation in most of the patients with their tooth pain, clinically no caries, no other visible any finding and x ray seem normal. Then, sometimes detailed case history helps to find out what is the cause.
sometimes we need to delve deep for the findings( in case of consistent pain ). We should not limit our diagnosis even if the reports are normal. Patient coming up with persistent pain should not be neglected.Because once they doubt healthcare, they lack trust. they should be given proper solution and treated accordingly. For example , if you go to a convenience store, you get items what you want and if you don’t, you’ll run to another store but the store won’t fail to provide your requirements. Similarly,Patient should be provided exactly with the best procedures the way they want it to be. By doing the same, we gain trusts not only of patients but this small mindset change is a win for entire healthcare sector.
