# What Are Oral Nicotine Pouches?

Oral nicotine pouches (ONPs) are small pouches placed inside the mouth, usually between the lip and the gums. They are often marketed as a safer alternative to cigarettes and other tobacco products because they do not involve smoking or burning tobacco. However, " **smoke-free" does not mean “risk-free.”**

The nicotine in these pouches is absorbed directly through the lining of the mouth and enters the bloodstream. Most pouches contain between **1.29 mg and 6.11 mg** of nicotine, which can lead to nicotine dependence and addiction over time.

# Effects on the Mouth

Regular use of nicotine pouches can cause several changes in the mouth, including:

- White or yellow patches at the site where the pouch is placed

- Thickened, rough, or leathery-looking oral tissues

- Gum irritation and soreness

- Blisters on the gums or oral tissues

- Dry mouth (xerostomia)

- Unusual sensations in the jaw or mouth

Long-term irritation of the oral tissues may increase the risk of developing serious conditions, including oral cancer, especially when combined with other tobacco-related habits.

# Effects on Gums and Supporting Tissues

Nicotine pouches can negatively affect gum health by:

- Causing gum recession (gums pulling away from teeth)

- Increasing attachment loss around teeth

- Triggering inflammation in the gums and supporting tissues

- Increasing the risk of periodontal (gum) disease

- Contributing to bone loss around teeth. Deeper gum pockets created by periodontal disease can harbor harmful bacteria, worsening oral health.

# Other Health Concerns

Research has identified more than **180** chemical compounds in some nicotine pouch products besides nicotine. Certain substances, such as **benzophenone, β-myrcene, and tobacco-specific nitrosamines (TSNAs),** have been linked to potential cancer risks.

# Other possible side effects include:

- Dental staining

- Stomach irritation and gastrointestinal problems

- Nausea or digestive discomfort

- Increased risk of nicotine addiction

# Impact on Healing After Dental Procedures

Nicotine reduces blood flow by constricting blood vessels. This can:

- Delay wound healing

- Increase the risk of infection after dental surgery

- Reduce the success of surgical flaps and grafts

- Slow bone healing after extractions, implants, or other procedures

# What Should Patients Know?

Although oral nicotine pouches are often promoted as a safer alternative to smoking, they can still have significant effects on oral and overall health. Regular dental check-ups and honest discussions about nicotine use can help identify early signs of damage and support nicotine cessation efforts.
