Long-acting antibiotics are antimicrobial agents with exceptionally long elimination half-lives. They maintain therapeutic drug concentrations for prolonged periods, allowing dosing intervals measured in days rather than hours. Additionally patients may require only one or two doses.

**Dalbavancin:** Dalbavancin is a second-generation lipoglycopeptide antibiotic effective against many Gram-positive organisms, including MRSA. It is approved for use in Acute bacterial skin and skin structure infections (ABSSSI) with half-life of approximately 8–10 days

**Oritavancin:** Another long-acting lipoglycopeptide with activity against a broad range of Gram-positive bacteria. Activity against MRSA, MSSA, streptococci, and enterococci (including some vancomycin-resistant strains). half-life of approximately 10 days

**Clinical Studies findings:** Clinical trials have shown that dalbavancin and oritavancin are **non-inferior** to conventional multi-day IV antibiotic regimens for treating acute bacterial skin and skin structure infections. Emerging evidence supports their use in selected bone and joint infections, though larger randomized trials are still needed.

**Advantages:**

- One or two IV doses
- Excellent adherence (minimal risk of missed doses)
- Reduced hospitalization
- Lower risk of catheter complications
- Suitable for outpatient management
- Improved quality of life
- Potential reduction in healthcare costs

Future of Antibiotics: scientist are exploring

1. Long-acting antibiotics for Gram-negative infections
2. Nanotechnology-based drug delivery systems
3. Implantable antibiotic depots
4. Long-acting combination therapies
5. Personalized antibiotic dosing using pharmacokinetic modeling

Long-acting antibiotics represent a significant advance in infectious disease management. By enabling treatment with a single infusion or just two doses, agents such as dalbavancin and oritavancin can shorten hospital stays, reduce the need for long-term IV access, and improve patient quality of life. As new long-acting agents are developed, these therapies are expected to play an increasingly important role in outpatient care and antimicrobial stewardship.

What do you think are limitations for these long acting antibiotics?

Long acting antibiotics are not pocket friendly. It can affect in long-term run also
