How should one start building it right straight from MBBS?
For LORs from US faculty, the best way is to do US clinical electives or observerships during MBBS. Perform exceptionally, be proactive, and show genuine interest in their work. Start early by researching hospitals that offer rotations to international students and politely reaching out to attendings.
Your hard work in these rotations directly leads to strong recommendations.

Find a hospital in the United States of America that offers clinical externship or rotation, then apply for a visa, pray to President Trump not to block the visa.
Arrive at the US, get checked at port of entry (do not bring rice, mangoes, seeds and pickles - they’re going to be taken away for sure).
Work diligently for the rotation and at least one attending will surely give you LOR.
