During my recent clinical postings, I observed a patient who was diagnosed with appendicitis and was receiving diclofenac injection along with pantoprazole.After administration of diclofenac injection patient complained of gastric discomfort and burning sensation. Upon further conversation with the patient, it was revealed that patient had a history of gastritis and diclofenac had aggravated his condition.
Although this may appear to be a simple Adverse Drug Reaction(ADR), but proper history taking could have helped avoid it.
Do you think history taking plays an important role in preventing a significant number of Adverse Drug Reactions (ADRs)?
I used love taking history during my mbbs days and internship, actually taking a proper history gets us to diagnosis of patients and also saves legally
History taking is absolutely substantial . It not just reflects on history but provides direction to treatment . History can help understanding the patient tendency and indeed prevent adverse drug reactions . Some doctors even ask about the foods you crave for the most as part of history taking. Although I do not know the relevance here. But it was something new to me.
Absolutely, as a dentist, I need to take an in-depth, comprehensive history of the patient. Noting down the medical history and medication history is essential because the patient might be using blood thinners (aspirin/Ecosprin), and this history is important before we start any tooth extraction or even deep scaling procedures.
Yeah it does play important role, Case History is important part of the treatment. History as name says it contains past medical issues, family history, trauma or accidents, allergies everything. Case history shows us what and how should be the further treatment of present illness.
