**SUMMARY**

Warfarin is an oral anticoagulant. It belongs to the vitamin K antagonist group. It works by blocking **vitamin K epoxide reductase**. This reduces the production of clotting factors II, VII, IX and X.
It is commonly used for atrial fibrillation, venous thromboembolism, and mechanical heart valves. The effect of treatment is monitored through PT/INR. For many indications, the usual target range is **INR 2.0–3.0.**
The **major adverse** effect is bleeding. Unusual bruising, nosebleeds, blood in urine, or black stools should not be ignored. Warfarin also has several drug interactions. New medicines, supplements, or herbal products should be discussed with a healthcare professional.
**Vitamin K** intake should remain consistent during treatment. Green leafy vegetables do not need to be completely avoided. Sudden dietary changes can affect anticoagulation control.
**Warfarin should not be stopped or adjusted without medical advice.** Patients should also inform their doctor or dentist before any procedure.
**KEY TAKEAWAY:**
Monitor INR. Watch for bleeding. Keep vitamin K intake consistent. Check for drug interactions.

**Question:**
**Which laboratory test is used to monitor warfarin therapy?**
