Telmisartan is an angiotensin II receptor blocker (ARB). It reduces aldosterone secretion, leading to decreased potassium excretion by the kidneys, which can result in hyperkalemia, especially in patients with kidney disease or those taking potassium supplements or potassium-sparing diuretics.

Hyperkalemia is caused by telmisartan use,bas the medicine can cause increased potassium loss through urine. The medicine works by modulating renin-angiotensin-aldosterone system (RAAS) pathway, and may cause hyperkalemia.

Telmisartan is an ARB which works by inhibiting aldosterone secretion. Aldosterone naturally promotes sodium reabsorption and potassium excretion in kidneys, suppressing aldosterone reduces potassium excretion which directly lead to elevated serum potassium level.

It is observed that Telmisartan reduces aldosterone levels, causing the kidneys to excrete less potassium, which can lead to elevated serum potassium.
