A 75-year-old woman presents with a 2-year history of gradual memory loss, difficulty with word-finding, and getting lost in familiar places. She has become increasingly dependent on her family for daily tasks. Her medical history includes hypertension and hyperlipidemia. Mini-Mental State Examination (MMSE) score is 20/30. What is the likely diagnosis, and what further evaluations would you conduct to confirm it?

It really sounds like Alzheimer’s disease is the most likely culprit. To be sure, we’d want to get a brain MRI to rule out other problems, check her blood work for anything treatable like vitamin deficiencies, and maybe do some more detailed memory tests.

Well as per my opinion it sounds like Alzheimer’s disease, with memory loss and difficulty in remembrance.
CBC, MRI and memory test are some simple test one can try to reach out for results.

can be Alzheimer’s disease since there is gradual memory loss, spatial disorientation, cognitive impairment etc.
Confirmation can be done by Neuropsychological Testing, MRI, CT, CSF biomarkers

Most likely duagnosis is alzheimer disease.
To confirm the diagnosis neuropsychological examination can be done along with CT, MRI and biomarkers

The case can relate to “Alzheimer’s disease”, considering the memory loss that happened in a gradual manner and other symptoms given. The MMSE score of that value indicates cognitive impairment at a moderate level. For confirming the diagnosis, tests should be done to map the complete cognitive profile and also MRI can be performed to evaluate the hippocampus region checking the extent of memory loss and its impact on brain. As she’s getting dependent upon other people, a complete functional assesment will also be needed here. The case should also be assessed in terms of prognosis as Alzheimer’s progresses quickly in most of the cases.
