Healthy gums don’t beg for attention but diseased gums demand it.

Recently I assisted in the management of an elderly patient with chronic generalized gingivitis and a localised periodontal abscess w.r.t 16, who presented with poor oral hygiene and severe pain. His pocket depth was 10mm.

An incision & drainage was performed to relieve infection, followed by periodontal therapy and later flap surgery for pocket elimination and healing.

This case was a reminder that gum disease starts silently but progresses aggressively when neglected.

Bleeding gums is rather a symptom. Apart from dental conditions it could be due to systemic illness.

Few causes are

1. Vitamin C deficiency (scurvy)
2. Vitamin K deficiency
3. Thrombocytopenia
4. Leukemia and other hematological disorders
5. Coagulation disorders (e.g., hemophilia, von Willebrand disease)
6. Diabetes mellitus
7. Pregnancy-related hormonal changes
8. Anticoagulant therapy
