**Have you ever wondered why two patients with similar symptoms of cough and breathlessness may have completely different diagnoses?**

Asthma and **Chronic Obstructive Pulmonary Disease (COPD)** are common respiratory disorders that often present with similar symptoms, including **cough, wheezing, chest tightness, and shortness of breath**. However, their causes, disease progression, and management are quite different.

1) **Asthma** is a **chronic inflammatory airway disease** with **reversible airflow obstruction**. It usually starts at a younger age and is triggered by **allergens, exercise, cold air, or respiratory infections**. Symptoms occur in episodes and respond well to **bronchodilators and inhaled corticosteroids**.

2) **COPD** is a **progressive lung disease** with **persistent airflow limitation** , most commonly caused by **long-term smoking or exposure to pollutants**. It usually develops after **40 years of age** , and symptoms gradually worsen over time. Management focuses on **smoking cessation, long-acting bronchodilators, and proper inhaler use**.

Recognizing these differences helps ensure **timely diagnosis, appropriate treatment, and better patient outcomes**.

**Conclusion:**
Not every patient with wheezing has the same disease. Identifying the underlying cause is essential for choosing the right treatment and improving long-term outcomes.

**Which investigation do you consider the gold standard for differentiating asthma from COPD, and why?**
