# A CLINICAL CASE DISCUSSION ON TAKAYASU ARTERITIS .

A 20-year-old female patient was diagnosed with Takayasu arteritis, an autoimmune condition where chronically, large blood vessel get inflamed.

Takayasu’s arteritis predominantly impacts **young women between the ages of 15 and 30.**

◇The exact pathophysiology is unknown, but it mainly involves combination of genetic susceptibility (strongly linked to certain human leukocyte antigens, mainly HLB-A \*53) and an environmental trigger (such as a viral or bacterial infection) that activates the immune system.  
◇ The stress of an infection usually causes the aortic endothelial cells to express high levels of HEAT SHOCK PROTEINS (HSP).  
◇The Killer T-cells (CD8+ cytotoxic T-lymphocytes) and Natural Killer (NK) cells infiltrate the outer layers of the artery and causes destruction of Elastic Lamina. The intimal thickening and fibrosis may cause Stenosis, Occlusion.

There are two distinct phases,

**STAGE 1** -Systemic Inflammatory Phase- Patients frequently report chronic fatigue, mild fever or night sweats, unexplained weight loss, and muscle and joint aches.

**STAGE 2** -Vascular Insufficiency Phase -This phase includes Limb Pain and weakness, Visual disturbances and RENOVASCULAR HYPERTENSION.

The patient was diagnosed based on the CT Angiogram which showed narrowing of blood vessel and had Renovascular hypertension (severe high blood pressure caused by the narrowing of renal arteries supplying the kidneys).  
Due to its spread to the renal arteries ( the main blood vessels supplying the kidneys),it leads to severe narrowing of the arteries , so named as Takayasu arteritis-induced renal artery stenosis (TARAS).  
Due to prolonged oxygen deprivation, it causes the affected kidney to permanently wither or lose its tissue mass and die which thereby leads to renal atrophy.  
The patient was admitted with creatinine level of 1.5 and CRP was also increased.  
Antibiotics were started and Anti-Hypertensives was started to maintain the blood pressure and Nephrectomy Procedure was performed.

Then she was haemodynamically stable and discharged after 2 days of observation.

The primary treatment was  
•High dose systemic corticosteroids like prednisone which helps to suppress inflammation.  
•Immunosuppressants like methotrexate or mycophenolate.  
•Biologic therapies like Tocilizumab are added to control the disease long term and lower steroid requirements.  
As the final step, Surgical interventions are made -If a critical artery becomes unsafely narrowed or an aneurysm forms, invasive measures such as **angioplasty** (widening the artery with a balloon/stent) or surgical bypass grafting may be required to restore proper blood flow.

While currently there is no cure , Takayasu’s arteritis is a treatable condition managed through a lifelong, multidisciplinary approach aimed at putting the disease into remission.
